Role and Responsibilities:
- A qualified professional, who directs, supervises,evaluatesandprovidesphysical therapy services to adult patients in the home,following physicians orders.
Essential Functions:
- Evaluate patient using diagnostic muscle, nerve, joint,and functional abilitytestsand measures.
- Prepare an assessment of the patients condition at the time of theinitialevaluation and perform periodic re-evaluations, develop a written problem list and plan of care including types and frequencies of treatment.
- Recommend modification of plan of careto physician,as needed.
- Document clinical notes on the date of the visit into medical record and keep all records and summariesup-to-date.
- Treat patients to address impairments and functional limitations, with a goal ofreachingmaximum functional independence.
- Instruct and/or recommend patients and their caregivers in the use of prosthetic, orthotic,and durable medical equipment (canes, walkers, wheelchairs, etc.).
- Include and educate thepatientandcaregiver(s)regardingtreatmentplanand goals.
- Attend physical therapy staff meetings and in-service presentations.
- Participate in case conferences.
- Participate in the development and periodic revision of the physicians Plan of Treatment.
- Assureappropriate safetypractices and maintenance of equipment.
- Other duties as assigned.
Education Requirements:
- Graduateof an accreditedPhysicalTherapy program.
Qualifications and Requirements:
- Current, valid Physical Therapy licenseissuedbyNYSDepartment ofEducation.
- One year of practical experience as a licensed physical therapist.
Work Environment:
- The Physical Therapist is primarily ina non-officesetting and may be exposed to outdoor conditions.
The working conditions are classified as medium work:
- Medium work – Exerting up to 50 pounds of force occasionally, and/or a negligible amount of forcefrequentlyor constantly to lift, carry, push, pull or otherwise move objects, including the human body. Medium work involves sitting and standing.
