Job Title: Registered Nurse (RN) – Field Nurse Case Manager

Reports To: Clinical Manager / Director of Nursing
Job Summary:
We are seeking a compassionate, experienced, and dedicated Registered Nurse (RN) to join our Home Health Agency team as a Field Nurse Case Manager. The ideal candidate will be responsible for managing and coordinating the care of patients in their homes, providing high-quality nursing assessments, care planning, and ensuring optimal patient outcomes. As a member of our interdisciplinary team, the RN Field Nurse Case Manager will work closely with patients, families, and healthcare providers to deliver comprehensive, patient-centered care.
Key Responsibilities:
Patient Care Management: Assess, plan, implement, and evaluate patient care in accordance with the plan of care developed with the interdisciplinary team.
Case Coordination: Serve as the primary point of contact for patients, families, and other healthcare providers. Coordinate care across multiple disciplines, ensuring continuity and high-quality outcomes.
Nursing Assessments: Perform thorough, accurate assessments of patients’ physical, emotional, and psychological status.
Care Planning: Develop, review, and update individualized care plans that meet the specific needs of each patient.
Patient Education: Educate patients and their families on disease management, medications, and any necessary treatments or procedures.
Documentation: Maintain accurate and timely documentation of patient care, including assessments, progress notes, and updates to care plans.
Collaboration: Collaborate with physicians, therapists, social workers, and other healthcare team members to ensure comprehensive care and positive patient outcomes.
Supervisory Role: Oversee and provide direction to home health aides or other support staff as required.
Compliance: Ensure all patient care complies with regulatory standards, policies, and procedures.
On-Call/Weekend Support: Participate in on-call rotation and weekend coverage as needed.

Qualifications:
Qualifications:
Current Registered Nurse (RN) licensure in the state of Florida.
Minimum of 1 year of clinical experience in home health, nursing, or related healthcare field preferred.
Strong knowledge of home health regulations, infection control, and documentation practices.
Ability to develop and implement comprehensive patient care plans.
Excellent communication and interpersonal skills, with the ability to effectively work with patients, families, and the healthcare team.
BLS certification required; ACLS or other certifications preferred.
Valid driver’s license and reliable transportation, as this is a field-based role.
Physical Demands:
Ability to travel to patient homes and perform assessments.
Ability to lift and move patients, equipment, and supplies as needed.
Must be able to work in various home environments, some of which may require bending, lifting, and standing for extended periods.
